SBA Intake Form

In order to become a Wyoming Women's Business Center (WWBC) client, you need to complete this form and send it to the office. You may send it as an email attachment or to the WWBC's mailing address (PO Box 764, Laramie, WY 82073). This form helps the WWBC staff to get to know you and the type of business assistance you need. The information contained within this form is entirely confidential.
